Wilber Care Center Board of Directors Meeting December 15th, 2025 Present: Randy Fulton, Board President, Nicole Lane, Administrator, Joyce Wusk, Board Member, Allison Suden, HR Coordinator, Stephanie Hroch, Board Member, Tracy Vitosh, Bus. Operations Coordinator, Greg Hasenohr, Board Member, Matt Ross, RHD consultant, Scott Albert, Board Member, Guests: None. Call to order: The regular monthly meeting was called to order at 5:30 p.m. by Randy Fulton, Board President. No visitors were present. Motion to approve meeting agenda with the correction for the next board meeting date and the November 2025 meeting minutes by Joyce Wusk. Motion carried. Current Updates: The resident Christmas party was a tremendous success. Many resident family members were able to attend, and the chime group was able to perform a variety of Christmas music for fellow residents and their families. This year, staff members will be purchasing resident Christmas gifts, allowing for a more personalized and meaningful gift-giving experience for each resident. Audit update- Lutz has not yet contacted us to schedule a meeting with the board. Once their review and finalization are complete, we will collaborate with them to set up a board presentation. There was a discrepancy identified between the MDS data and the weighted days, which appears to be related to census reporting. The business office is working to reconcile this. Current Census: The current Skilled Nursing census is 28, consisting of 5 Medicare A residents, 10 Medicaid residents, and 13 private-pay residents. One admission is expected later this week. Assisted Living census is 29 in-house, with 1 resident on bed hold, for a total census of 30. There will be a new admission later this week. Review of Personnel Openings: The Director of Nursing position remains open. Nebraska is currently experiencing a shortage of RN’s resulting in difficulty filling the DON position. The assisted living night shift opening has been filled which will eliminate the need for agency staff in assisted living. Full-time and part-time med aids have been hired for the evening shift in assisted living as well. A part-time CNA and a part-time med aide have been hired for the nursing department. The med aide is currently enrolled in the LPN program and will graduate in May. An on-call position was filled in the housekeeping department and a full- time position was filled in the dietary department. Rural Health Development Update: RHD nurse consultant Vicki Lapent continues to offer support to the nursing personnel in the absence of a full-time DON. Kristal Thorell continues to work with the business office on Medicare and Medicaid billing. General financials: Financial Statement: Medicaid and private revenue increased for the month of November. Medicare A and B revenue decreased. CD income trended down as interest rates fell compared to previous months. Aging of Accounts: The facility continues to work on decreasing aging of accounts with focus on tracking past-due balances and providing training on which amounts are appropriate to adjust off. CD activity: Three CDs were able to be rolled over at their current banks. Motion to approve the income section made by Greg Hasenohr, motion carried. General Expenses: The holiday bonus was paid out in the final November payroll, which resulted in increased wages in several departments, while administrative wages remained steady. Worker’s compensation expenses increased again in November due to further auditing. Agency costs for both nursing and assisted living continue to trend down due to filling open positions. Assisted living education expenses increased as Assisted Living Coordinator Karis Bredemeier attended assisted living administrator training. Dietary wages increased due to training a new staff member. Activities wages and supplies were higher, while housekeeping wages were steady. Social services and maintenance wages were higher as a result of the holiday bonus. Utility costs were up due to the colder weather. Overall, the facility experienced a loss for the month of November. Bills: One question was asked and answered regarding what services are rendered by Arthur Gallagher; They are the facilities insurance company. Motion made by Stephanie Hroch to approve the expense section, motion carried. Old Business: Office remodel – Quotes for the HVAC system has not yet been received. Maintenance Supervisor Levi Lane followed up and was informed that they are not ready at this time. Personnel project- Administrator Nicole Lane and Board President Randy Fulton are working on a personnel project reviewing all current open positions and wages for each position. NEW BUSINESS: Revalidation- The IRS form 2848 was reviewed and updates were made. Board member recommendation renewal- Board member Stephanie Hroch agreed to remain on the board for another five-year term. All board members agreed to this recommendation.
